VITAL STATISTICS:

A vital statistic is a certified record of a Birth, Death, Marriage, Civil Union or Domestic Partnership which has occurred here in the Borough of Emerson.

How do I apply for a civil union license?

In order to apply for a Civil Union License, the partners must have a witness18 years of age or older.  The partners must have a government issued identification like a Driver’s License.  If the partners do not have a Driver’s License they can present passports with a copy of a bank statement/utility bill with their current address listed.   There is a minimum of a seventy-two (72) hour waiting period between the time of application and the issuance of the license.  Once issued, the license is valid for thirty (30) days.  The license fee is $28.  One or both partners must be a resident of Emerson.  If the partners are out of state residents, they should apply in the municipality where the ceremony will take place.  Please call Northwest Bergen Regional Health Commission at 201-445-7217 ext. 0 to set up an appointment.   For a link to the forms required please go to:  www.nwbrhc.org

What do I need to obtain a certified copy of a vital statistics record?

The individual should contact the municipality where the event took place.  The individual will complete an application for a certified copy of record and show a valid form of identification, such as a driver's license or passport and utility bill/bank statement.  The fee is $25.00 (each additional copy is $2.00 if ordered at the same time).  For a link to the forms required please go to:  www.nwbrhc.org

 

WHO CAN OBTAIN A CERTIFIED COPY?

The following is a list of individuals that may request a copy of a Vital Statistic. Persons requesting a record must provide acceptable identification. There are NO exceptions.

  • Subject of record (Birth, Marriage, Domestic Partnership) - See note*
  • Surviving Spouse (Death)
  • Current Spouse
  • Subject's Parents
  • Child of Subject
  • Grandchildren of the subject
  • Siblings - See note*
  • Legal Guardian
  • Legal Representative
  • Court Order

NOTE:

Female applicants that are married and has assumed their husband's surname must also provide a certified copy of their marriage certificate when requesting a copy of their own birth certificate.

Individuals requesting a sibling's birth certificate must also provide their own birth certificate in addition to the other identification requirements.
